Over the course of the last few months our team has had staff vacancies that left many of youth uncertain of case management next steps. After working diligently to fill the gaps we are in a position to provide youth with great support yet again.

Over the course of the last few months our team has had staff vacancies that left many of youth uncertain of case management next steps. After working diligently to fill the gaps we are in a position to provide youth with great support yet again.
June 2022 Updates:
- We welcome Amos Park, Jordan Lavoie, and Rayanne Banaga, as our Youth Resource Workers.
- All service users are in the process of being assigned a new Case Manager.
- Case management, with scheduled goal-oriented meetings, resumes in full capacity.
- Once assigned, Youth Resource Workers will reach out to service users to complete an assessment, and sign a service agreement to resume ongoing case management. Service users are encouraged to keep an eye on their inbox and spam for an email from Amos, Jordan, Rayanne, or Susie.
Previous Updates:
- Adanyi Phillip has moved into the role of Program Coordinator – Housing and GPS.
- Courtney Niven has moved into the role of Program Coordinator – Mentorship and Peer Leadership.
- We welcomed Susie Mensah, who joined us as a Youth Resource Worker (YRW).

Are you a healthcare provider or community worker who’d like to refer someone to our GPS service? Please download our External referral form, fill it out and email it to us at gps@sherbourne.on.ca.
Withdrawing from case management
If at anytime youth do not wish to be contacted and would like to withdraw from case management, they can send an email to GPS@sherbourne.on.ca. Once contact with a case manager has been made, withdrawal from service can be done through the assigned YRW. If you know of someone who was receiving case management but did not receive an update email, please have them contact GPS@sherbourne.on.ca.
