Supporting Our Youth’s (SOY) case management program called Goal Planning Service (GPS) has reopened. Goal Planning Service or GPS is a case management program staffed by Youth Resource Workers who support participants one-on-one with goal planning, capacity building and making connections to needed resources to meet established goals. The program is suited for youth who are ready to agree to a service plan and work towards achieving SMART goals. Referrals are accepted on an ongoing basis.

If you are a health care provider or community worker who’d like to refer someone to our services, please check out our listing on the Ocean Healthmap. Withdrawing from case management
If at anytime youth do not wish to be contacted and would like to withdraw from case management, they can send an email to GPS@sherbourne.on.ca. Once contact with a case manager has been made, withdrawal from service can be done through the assigned YRW. If you know of someone who was receiving case management but did not receive an update email, please have them contact gps@sherbourne.on.ca
